Insurance and Billing

Services may be provided in this health care facility by the facility as well as by other health care providers who may separately bill the patient and who may or may not participate with the same health insurers or health maintenance organizations as the facility.

Patients and prospective patients may request from this facility and other health care providers a more personalized estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider. At the time of your visit, you will need to provide the front desk with your current insurance information, including your insurance card(s) to be copied. We will perform courtesy billing for you, provided you have given us all pertinent billing information. You are responsible for full payment of any co-payments, co-insurance and deductibles required by your insurance company.

Your insurance coverage is an agreement between you and your insurance company. Financial responsibility rests with the patient for deductibles, co-insurance and non-covered services.

In addition to your bill from the center, you may receive a bill from other entities. Click here for a list of possible service providers that may charge you.

If your insurance requires authorization for an office visit, please contact your referring physician for that authorization prior to calling for an appointment.

The Cost of Your Procedure

Because you are responsible for costs for covered healthcare services until you reach your deductible, you should comparison shop if you elect to have a nonemergency procedure.

You can use tools like Healthcare Bluebook and the Medicare Procedure Price Lookup (for Medicare patients).  Many health insurance websites also provide information on where to find in-network services.
